Good Faith Estimate (No Surprises Act)
Good Faith Estimate Notice: Under the No Surprises Act, clients who are uninsured or who choose not to use insurance have the right to receive a Good Faith Estimate of the expected cost of mental health services. A Good Faith Estimate outlines anticipated charges based on the services reasonably expected to be provided. This estimate is not a contract and does not obligate you to receive services.
You may request a Good Faith Estimate:
Prior to scheduling services
At any time during treatment
If you receive a bill that is $400 or more above the amount listed in your Good Faith Estimate, you may dispute the charges.
For questions or to request a Good Faith Estimate, please contact the practice directly.
